At this time, Creative Pathways offers three services based around quality living and independence.
Individualized Supported Living (ISL)
If you, a family member, or friend believe that you or someone else might qualify for services, please contact us so that we can answer any questions that you might have.
Living in an ISL enables individuals with disabilities to be fully integrated in communities. ISL services provide individualized supports, delivered in a personalized manner, to individuals who live in homes of their choice. Individuals receiving ISL supports may choose with whom and where they
live, and the type of community activities in which they wish to be involved.
Personalized Support Plans are created and customized for the individual in collaboration with the agency, the individual receiving services, support coordinators, and guardians (if applicable).
Homes are staffed by Creative Pathways up to 24 hours a day 7 days a week depending on the individuals needs in order to best support the individual in all aspects of daily life.
What is the Host Home Program?
Host Home is a program provided by the Department of Mental Health and shared with the Children’s Division. The program is designed to keep individuals in a traditional home setting, where they will be a part of a positive environment. Host home families act as advocates, friends and mentors who not only share their home with people with developmental disabilities, but also help them engage in community life, teach them new skills, help them make friends and enrich their lives.
On average, a host home receives an approximate compensation of $2517.20-3550.80 monthly but may vary based on individuals needs. A host home can qualify to host up to 3 individuals.

Who can qualify to offer this service?
A host home can be a family, single person, college student, single parent, etc.
Creative Pathways Availability
Since we are an agency based out of Linn County, designed to service our under-served area, we are available 24/7 to care for any needs of both our supported individuals and host home families that might arise.
What is Personal Assistance?
This service is designed to provide an individual requiring some but not total support services, the personal assistance they need to maintain their personal independence.

Who provides Personal Assistance?
Creative Pathways employs staff to provide services to the individuals we support. Staff are trained to provide personal assistance with daily activities, general supervision, and/or protective oversight.
What is Support Broker?
Individuals requiring the Support Brokering service are considered Self-Directed. Self-Directed individuals manage their own services, outside of the support service agency.
What does Creative Pathways do for Self-Directed individuals?
Creative Pathways will act as support broker for these individuals. This entails providing information and assistance to the Self-Directed individual in the management of their administrative needs such as:
- Staffing needs
- Problem Solving
- Conflict Resolution
